Authorization to service
I authorize Cruise the Creek to inspect, diagnose, and perform the requested service or repairs on my e-bike, including any reasonable test rides needed to diagnose or verify the work.
Pre-existing condition
E-bikes brought in for service may have pre-existing wear, damage, or hidden faults. Cruise the Creek is not responsible for pre-existing conditions, or for unrelated issues discovered or developing during diagnosis or repair.
Personal property & accessories
I will remove valuables and removable accessories before drop-off. Cruise the Creek is not responsible for lost, stolen, or damaged accessories, batteries, or personal items left on or with the bike.
Estimates, parts & fees
Diagnostic and parts fees may apply. Cruise the Creek will contact me for approval before performing work beyond the requested service or exceeding any agreed estimate.
Batteries & electrical
I acknowledge that e-bike batteries and electrical systems carry inherent risks, and I represent that the battery and charger provided are the manufacturer's or a compatible, undamaged unit.
Pickup & abandoned property
I will pick up my bike within 30 days of being notified that service is complete. Bikes left beyond 30 days may incur storage fees, and bikes left beyond 60 days may be treated as abandoned and disposed of as permitted by Ohio law.
Release of liability
To the fullest extent permitted by law, I release Cruise the Creek, its owners, and staff from liability for loss or damage arising from this service, except loss or damage caused by their gross negligence or willful misconduct. I confirm I am the owner of this bike, or am authorized by the owner to approve this work and sign this waiver.
